Allegations: The Resident was admitted to Wood Lake Health and Rehabilitation Center where he developed multiple pressure ulcers which resulted in MRSA, osteomyelitis and an amputation of the right leg above the knee. He wrongfully died on November 25, 2016.

Allegations: The Resident was admitted to Wood Lake Health and Rehabilitation Center where the Resident failed to provide the Resident with adequate and appropriate care which resulted in at least six falls, the last of which resulted in a left hip fracture that required surgical repair.

Allegations: The Resident suffered a catastrophic fall on October 28, 2013 at Wood Lake Health and Rehabilitation Center resulting in a subdural hematoma and significant injuries that caused his untimely and wrongful death on December 10, 2013.
RESULT: Case disposed by Settlement on March 6, 2017.
April 24, 2015: MAUS, VICKI V 6414 13TH ROAD SOUTH OPERATIONS LLC
Allegations: The Resident was admitted to Wood Lake Health and Rehabilitation Center on August 24, 2013 and remainder there until October 2, 2013. He was admitted again on October 30, 2013 through February 28, 2014. The Facility breached duties owed to the Resident resulting in the worsening of decubitus ulcers of his sacrum and left heel, a rash, multiple falls, malnutrition, a urinary tract infection, multiple bruises and lacerations, a MRSA infection, and pneumonia infection. These injuries caused the wrongful death on March 20, 2014.
RESULT: Case sent to Arbitration on August 16, 2016.
February 21, 2014: SINGER, SHARON V WOOD LAKE HEALTH CARE ASSOCIATES LLC
Allegations: While at Wood Lake Health and Rehabilitation Center in 2007, the Resident developed aspiration pneumonia and subsequently went into respiratory failure which required intubation. The Resident was transferred from the Facility to Palms West Hospital for urosepsis with abnormal liver functions tests.

As a result, the Resident wrongfully died on December 1, 2009.

Allegations: An individual was injured after falling due to the careless and negligent manner in which the floors were maintained in the hallways at Wood Lake Health and Rehabilitation Center. The individual slipped on water and fell causing her bodily injury.

Allegations: The Resident was admitted to Wood Lake Health and Rehabilitation Center on March 17, 2008 where she remained until being discharged on April 29, 2009. The Facility breached duties owed to the Resident which caused numerous skin issues including a dermatological condition requiring an anti-itch medication, decubitus ulcers to her left inner buttocks, mid-back and left heel, infections including pneumonia and multiple UTIs and sepsis from the IV line, weight loss, uncontrolled blood sugar levels and dysphagia.
RESULT: Case disposed on March 11, 2013.
