October 6, 2015: LANG, FRANCINE R V ACTS RETIREMENT LIFE COMMUNITIES INC
Allegations: On May 14, 2014, the staff at Willowbrooke Court at St. Andrews Estates failed to comply with wheelchair policies in providing safe and reasonable wheelchair service to the Resident. Specifically, the staff failed to secure the Resident in her wheelchair and pushed the Resident in her wheelchair in such a way as to cause her to suffer an injury to her lower extremity which caused pain and suffering prior to her death on September 14, 2014.
RESULT: Case disposed on June 5, 2017.
August 10, 2015: BOURQUE, KATHY V ACTS RETIREMENT LIFE COMMUNITIES INC
Allegations: The Resident was admitted to Willowbrooke Court at St. Andrews Estates on January 12, 2015 where the Facility failed to implement proper fall precautions and failed to provide an appropriate level of supervision and assistance. As a result, on January 22, 2015, the Resident suffered a preventable fall resulting in a femoral neck fracture.
RESULT: Case disposed by Settlement on May 11, 2016.
December 16, 2014: MOLONY, DOTTIE V ACTS RETIREMENT-LIFE COMMUNITIES INC DBA ST ANDREWS ESTATES
Allegations: On July 11, 2013 the Resident was in the dining room of Willowbrooke Court at St. Andrews Estates when another resident operating a motorized scooter struck the Resident causing injuries. The Facility did not warn or supervise the use of motorized scooters on the premises.
RESULT: Case disposed on March 9, 2016.
