Allegations: While at Whitehall Boca Raton on February 2, 2015, the Resident sustained a fall while transferring to her bed that resulted in significant and severe injuries.

RESULT: Case is on-going.

June 6, 2016: HARDIN, DAVID L V WHITEHALL OPCO, LLC
Allegations: During the Resident’s stay at Whitehall Boca Raton, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of severe pressure sores on her left buttock, right anterior forearm, and left lateral lower leg. Due to these injuries, the Resident wrongfully died.

Allegations: The Resident was admitted to Whitehall Boca Raton when on April 12, 2015 she suffered sudden swelling, hemorrhage and a large hematoma at the Facility as a result of the overmedicating her which resulted in serious bodily injuries and death on April 25, 2015 due to the Facility’s negligence.

RESULT: Case disposed on July 10, 2017.

Allegations: The Resident was admitted to Whitehall Boca Raton on February 9, 2014 for rehabilitation after a hip fracture from a prior fall. She also suffered from periodic confusion, generalized anxiety disorder and hypertension and was thus a fall risk. Proper preventative fall techniques were not put in place which allowed the Resident to suffer a debilitating fall on February 14, 2014. The Resident rang her call bell to use the restroom and the staff failed to respond. The Resident attempted to use the restroom and fell, fracturing her hip.

RESULT: Case was disposed by Settlement on October 27, 2015.

Allegations: An individual was injured when she slipped and fell on a liquid that had been allowed to accumulate near the entrance/exit after dark. The fall caused substantial and permanent injuries.
