Allegations: While at The Waterford, the staff failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering falls and unexplained injuries and suffering a urethral tear.

RESULT: Case disposed on May 31, 2016.

June 15, 2015: REINKE, LOIS V LIFESPACE COMMUNITIES INC
Allegations: On September 4, 2014, The Waterford owned and operated a shuttle bus in Juno Beach, Florida. The resident was a passenger in the shuttle bus. The driver of the bus negligently operated and maintained his motor vehicle so as to cause it to make a sudden and fast left turn, causing the passenger to fly off of her seat causing serious injuries.

RESULT: Case was disposed of by way of settlement on February 5, 2016.
