During the time he was a Resident at the Facility, he developed multiple bed sores, specifically a pressure sore of the left heel which was rated a Stage IV by the time he was discharged which ultimately led to a left knee above the knee amputation.

She was admitted for rehabilitation; however, on February 12, 2009 the Resident fell severely injuring her left knee which had recently been operated on which caused severe injuries, aggravation of preexisting conditions, and infection which proximately caused her death on or about March 7, 2009.
