April 13, 2015: GOLDSTEIN, STEPHEN V SENIOR LIVING PROPERTIES V LLC
Allegations: The Resident was admitted on November 1, 2013 to Savannah Cove. The Resident suffered a preventable fall on March 19, 2014 due to the Facility’s negligence for failing to properly assess and treat the Resident after the fall, further exacerbating the injuries and resulting in the Resident’s death.

RESULT: Case disposed on March 8, 2016.

September 29, 2014: ESTATE OF RUTH C WINSLOW V SENIOR LIVING INVESTORS I LLC
Allegations: On October 8, 2012, the Resident was admitted to Savannah Cove where she remained until she was discharged on March 28, 2013. The Facility breached duties owed to the Resident which caused injury including multiple falls resulting in skin tears to her right lower leg and right elbow, a skin tear to her right lower led, red sacrum, and a left hip decubitus ulcer with necrosis. The Resident wrongfully died due to these issues on May 17, 2013.

RESULT: Case sent to Arbitration on April 25, 2017.

May 16, 2013: GOLDEN, MARYANN V SENIOR LIVING PROPERTIES V LLC
Allegations: The Resident was admitted to Savannah Cove on February 10, 2012 where she later developed a sacral decubitus ulcer as well as an unstageable decubitus ulcer on her left heel. On March 10, 2012 the Resident was being transferred from her bed onto a toilet when the nursing staff dropped her onto the floor and she struck her right knee and head. On March 12, 2012, the Resident’s daughter visited her mother who noticed her mother’s knee was deformed. On March 14, 2012, the Resident was transferred to Good Samaritan Hospital where it was determined she had a broken femur, was black and blue in and around the side of her face, she had a urinary tract infection, pneumonia, a recent stroke and decubitus ulcers on her heels and sacrum. The Resident could not be taken to surgery because of her malnutrition and the physician did not feel that she would survive. After hydrating her and treating her infections, the Resident underwent surgery to repair her broken femur which involved internal fixation and insertion of a rod from the hip to the knee. She was never able to walk again and became a prisoner in her own body.

RESULT: Case disposed on June 23, 2014.

March 26, 2010: BERGER, FERN S V SEIOR LIVING MANAGEMENT CORP
Allegations: Savannah Cove filed an abuse report to Florida Department of Children and Family services accusing the Resident’s family member of abusing the Resident. The family claimed the abuse report were false and caused them to hire an attorney to refute the claims.

RESULT: Case disposed on October 12, 2010.
