Allegations: The Resident was admitted to Rehabilitation Center at Jupiter Gardens LLC when on January 5, 2016 when the Resident suffered a preventable fall due to the Facility’s negligence which resulted in serious bodily injuries including a fractured hip which required major open surgery. Subsequent to the surgery, she suffered significant blood loss and required transfusions. After returning back to the Facility, the Resident developed skin breakdown due to the Facility’s negligence including multiple Stage III pressure sores to the buttocks. The Resident also lost a significant amount of weight following the hip fracture and declined to the point where she was admitted to hospice on April 26, 2016 where she died on May 6, 2016, only four months after the fatal fall at the Facility.
