Allegations: While at Regents Park Nursing & Rehabilitation Center in Boca Raton, Florida, the Resident was transported to the facility from Boca Raton Regional Hospital. Soon after admission certain identifiable significant findings were documented in the Nursing Home Chart regarding the Resident’s global medical condition. It showed clear signs of decompensation including alarming vital signs with a documented blood pressure of 90/45. The facility failed to provide adequate health care consistent with the resident care plan and/or physician orders. As a result, the Resident sustained bodily injury resulting in death.

November 18, 2016: CATAPANO, FRANK V REGENTS PARK BOCA RATON INC
Allegations: During the Resident’s stay at Regents Park Nursing & Rehabilitation Center from January 9, 2015 through January 27, 2015, he was admitted for rehabilitation following a fall and hip surgery. The staff failed to monitor the Resident after he suffered a fall and a head injury and subsequently failed to monitor the Resident to the extent that he suffered an additional fall in his room after being left unattended. As a result, the Resident was injured.

August 24, 2016: CURTISS, JANET V REGENTS PARK, INC.
Allegations: The Resident was admitted from Boca Raton Regional Hospital to Regents Park Nursing & Rehabilitation Center. The Resident was assessed as a high risk for falls upon her March 16, 2015 admission and she required extensive assistance with walking and supervision with toileting. On March 20th, just for days after her admission to the facility, the Resident suffered a fall and was found on the floor by staff. Proper fall precautions were not put in place after this fall. On April 4, 2015, the Resident suffered another fall off of the toilet when she was left unsupervised. This fall went unreported to the Resident’s family and physicians. The Resident’s family took the Resident out to enjoy the day together when the Resident advised the family she fell and was in a lot of pain. The Resident’s family advised the facility of the fall and that she was hurt. The facility continue to deny that the Resident fell and attributed the misunderstanding to the Resident’s dementia. Eventually the Resident was taken to Boca Ration Regional Hospital where they confirmed she suffered a T-12 compresion fracture in her thoracic spine and underwent a T-11 kyphoplasty spinal surgery and led to the Resident’s wrongful ldeath. The facility later confirmed that she did suffer a fall on April 4, 2015.

Allegations: Regents Park Nursing & Rehabilitation Center failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of wounds. As a result, the Resident wrongfully died on June 14, 2016.

Allegations: Prior to December 24, 2013, Regents Park Nursing & Rehabilitation Center was aware that the Resident had a risk of falling and was required to be placed in a bed with a lowering mechanism that contained a functioning alarm system that would active and alert staff if the Resident got out of bed voluntarily or otherwise fell out of bed. The facility placed the Resident in a bed that was raised and did not contained a functioning alarm system. On December 24, 2013, the Resident fell from his bed and was found on the floor of his room, next to his bed, and the staff was not alerted because they did not have a functioning alarm system on his bed. The staff did not call 911 but rather lifted him off of the floor and placed him in an adjacent bed. The Resident waited in substantial pain and it was not until eleven and one-half hours after that he was finally transferred to Delray Beach Medical Center for evaluation and treatment. At Delray Medical Center it was determined that the Resident sustained a broken hip either from the fall or being improperly moved, which required hip replacement surgery.

Allegations: The Resident was transferred to Regents Park Nursing & Rehabilitation Center from Delray Medical Center after undergoing a left total hip replacement. During the early morning of August 22, 2009, the Resident was in her room, in bed, and needed to use the restroom. After pressing the call button numerous times over an extended period, none of the staff came to assist the Resident to the restroom. The staff left a portable toilet next to her bed with the bedrails low. The Resident attempted to step out of bed but the Resident’s hip was unable to bear any weight and she immediately fell and felt immediate pain to her right wrist and lower back. As a result, she suffered a fracture to her right wrist and cannot participate in her normal activities without substantial pain and discomfort.
RESULT: Case was disposed by Judge on September 24, 2013.

Allegations: The Resident required the need for increased supervision while at Regents Park Nursing & Rehabilitation Center. The Resident was injured while under the care of the facility. She was then transferred to Boca Raton Community Hospital with a complaint of left hip pain and was found to have a subcapital fracture of the left hip and had to undergo a left hip bipolar hemiplasty and later died a little over two months later.

Allegations: While at Regents Park Nursing & Rehabilitation Center, the Resident was provided with personal assistance in ambulating to the restroom. Instead of remaining with her, the facility’s employees left her unattended in the restroom instructing her to signal the attendance buzzer when she was ready to return to bed. When she was ready to return to bed, the Resident activated the call buzzer which went unattended for approximately 30 minutes. The Resident attempted to call out for assistance which was not forthcoming and realized that the employees were not coming to assist her and she attempted to ambulate from the toilet to the sink when her knee buckled and she fell to the floor sustained serious personal injuries.
