Allegations: While at Lakeside Health Center, the Resident suffered from injuries, including development of skin compromise and deterioration which progressed to decubitus ulcers. Lakeside Health Center failed to provide the Resident with a safe environment, failed to provide adequate skin assessments, failed to provide adequate and appropriate skin care, failed to prevent the development and worsening of decubitus ulcers, failed to properly document decubitus ulcers, failed to monitor significant signs and symptoms in Resident’s change in condition, failed to monitor significant signs and symptoms of infection, and failed to provide adequate and appropriate hygiene. As result, the Resident suffered from an unstageable decubitus ulcer to the bone on her buttocks/sacrum, multiple ulcers on her toes and infections of her decubitus ulcers.
