Allegations: The staff and employees at Glades Health Care Center failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deteriorating of pressure sores and suffering significant weight loss and malnutrition. Allegations: In January 2008, the Resident was admitted to Glades Health Care Center where she was non ambulatory and required maximum assistance for all activities of daily living. On January 12, 2008 the Resident was found unresponsive and breathing heavily requiring her to be transferred to Glades General Hospital. While at the emergency room, a CT scan of the brain revealed a large left-sided subdural hematoma which measured 2.3 cm in thickness with mass effect and a midline shift to the left. The CT also revealed a subfalcine herniation, a compressed and displaced left ventrical, left uncal herniation, a subarachnoid hemorrhage in the cerebella hemisphere and a small right temporal and subdural hematoma. Surgery was not an option and the patient expired two days later on January 14, 2008 without regaining consciousness.
