Allegations: While at Darcy Hall of Life Care, on January 23, 2016, the Resident was independently ambulatory and unsupervised when she attempted to exit a set of automatic slide doors leading to the courtyard area at Darcy Hall of Life Care. The automatic slide doors were in a state of disrepair and/or were malfunctioning. As the Resident attempted to exit, the automated doors closed on her, striking her in the head and knocking her to the ground where she violently struck the back of her head. She was taken by ambulance to St. Mary’s Medical Center where was diagnosed with a traumatic brain injury.

On September 14, 2015, the Resident was transferred from Darcy Hall of Life Care to Good Samaritan Medical Center where it was noted she was suffering from several life threatening medical conditions, including but not limited to, severe dehydration, her sodium level was extremely high being noted as hypernatremia, she had multiple open wounds located on her left thumb, right heel, left heel, a stage IV decubitus ulcer on her sacrum, extremely elevated white blood cells count reflecting an acute and active infectious process in her body and left lower lob pneumonia that was acquired at Darcy Hall of Life Care.

Allegations: During the Resident’s stay at Darcy Hall, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering falls and unexplained injuries. The Resident suffered a catastrophic fall which resulted in her suffering a fractured femur and wrongfully died on July 25, 2014.

Allegations: On January 3, 2011, the Resident was admitted to Darcy Hall and was promised that a team of specialists would be coming to the facility to monitor and treat the Resident’s right foot but this never occurred. During the Resident’s 16 day stay at the facility, the Resident became dehydrated and she developed gangrene throughout her right foot and leg. Due to her gangrene leg, she was admitted to a hospital where they performed an above the knee amputation on January 19, 2011 and the Resident later died on March 29, 2011.
RESULT: Case went to trial and the Resident obtained a Judgment for $500,000.00 on October 25, 2012.

Allegations: The Resident was admitted on January 25, 2007 and resided there until March 4, 2007. On April 1, 2007, the Resident died due to the negligent care and supervision of the Facility. The Resident fell out of his wheelchair on several occasions including February 3, 2007 and February 5, 2007. The Resident had skin integrity issues resulting in pressure sores and cellulitis. Due to these conditions, the Resident wrongfully died.
