Allegations: The Resident was admitted to Coral Bay Healthcare and Rehabilitation from June 26, 2013 to July 20, 2013. The Resident was later admitted to JFK Medical Center where he was deemed unsalvageable from sepsis due to gangrene legs. These injuries resulted in the Resident’s death.
RESULT: Case was disposed on January 5, 2016.

Allegations: The Resident was admitted to Coral Bay Healthcare and Rehabilitation on December 21, 2011 with no signs and symptoms of skin breakdown noted. On February 10, 2012 the Resident was admitted to JFK Medical Center wherein he was septic from an unstageable deep tissue injury/wound to the sacrum.

Allegations: The Resident was admitted to Coral Bay Healthcare and Rehabilitation where the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deterioration of thrombosis in the leg with accompanying occlusion and ischemia. Due to these injuries, the Resident wrongfully died.

Allegations: The Resident was admitted to Coral Bay Health Care and Rehabilitation in September 2009 until he was discharged on December 24, 2009. The Facility breached duties owed to the Resident which resulted in the extreme worsening of a coccyx decubitus ulcer which resulted in the Resident’s death.

Allegations: The Resident was admitted to Coral Bay Health Care and Rehabilitation on June 24, 2011 where she remained until being discharged on June 27, 2011. The Facility breached duties owed to the Resident which resulted in multiple falls, multiple infections including food/vomit pneumonia, septicemia, severe sepsis, acute respiratory failure, esophageal reflux and HTN. Due to these injuries, the Resident wrongfully died.
RESULT: Case was disposed on April 7, 2015.
March 22, 2010: SELTMAN, RANDI D V CORAL BAY HEALTH CARE ASSOCIATES LLC
Allegations: The Resident was at Coral Bay Health Care and Rehabilitation on March 23, 2006 where the employees failed to have the bed railings secured such that the bed being occupied by the Resident was unsecured. As a result the Resident rolled to the side of the bed and fell resulting in serious and permanent injuries.
RESULT: Case was disposed by the Judge on November 12, 2013.
January 27, 2010: OZAROW, ELAINE V CORAL BAY HEALTH CARE ASSOCIATES LLC
Allegations: The Resident was at Coral Bay Health Care and Rehabilitation on June 10, 2009 and a social worker was attempting to get out of bed unassisted. No preventative action was taken to prevent the Resident from getting out of bed unassisted and/or from him suffering injury. The Resident was transferred to Palms West Hospital where he was diagnosed with a broken neck which required surgery. The Resident died as a result of complications of multiple fractures due to the fall at Coral Bay Health Care and Rehabilitation.
RESULT: Case was disposed on July 15, 2010.

Allegations: The Resident was at Coral Bay Health Care and Rehabilitation from April 5, 2009 through April 20, 2009. The Facility failed to provide adequate skill care and services to the Resident which resulted in a decline in nutritional status, failure to note urine output, decline in medical condition. This caused the Resident to due on April 21, 2009.

Allegations: On December 8, 2006, the Resident was placed in an knee immobilizer with medical orders that all range of motion activities to the Resident’s left knee region be eliminated. The physical therapy progress notes document that range of motion activities were performed on the Resident’s left knee in contravention to medical orders. This resulted in severe damages including deformity, loss of all mobility and range of motion in her left knee.
