September 21, 2016: SANDRA HOOVER, as Power of Attorney for GUSSIE GECHTMAN V SOVEREIGN HEALTHCARE OF WEST PALM BEACH LLC
Allegations: While at Boulevard Rehabilitation Center on February 18, 2016, the Resident was to receive rehabilitation following a recent hospitalization for a suspected transient ischemic attack. The Resident required fall preventative measures be put into place to prevent future falls and she needed increased supervision with mobility. Despite this fall risk classification, proper fall preventative measures were not in place and the Resident suffered a debilitating fall in the bathroom on March 2, 2016 and suffered severe injuries and fractures.

July 19, 2016: ACOSTA, LINDA A V SOVEREIGN HEALTHCARE OF WEST PALM BEACH, LLC
Allegations: During the Resident’s stay at Boulevard Rehabilitation Center, the Resident developed an infected pressure sore and lost weight due to poor nutrition while being attended to by employees of the Facility. The Facility failed to adequately observe and care for the Resident to insure her safety and well-being which resulted in complication and her death. On July 23, 2014, the Resident died as a result of sepsis caused by the development of coccyx sores, improper nutrition and hydration.

June 27, 2016: JESKEY, HELEN V SOVEREIGN HEALTHCARE OF WEST PALM BEACH, LLC
Allegations: The Resident was admitted to Boulevard Rehabilitation Center on September 23, 2014. On September 24, 2014, the Resident suffered a traumatic incident to the Facility’s negligence, which resulted in serious bodily injuries including an acute comminuted humerus fracture. Less than one week later, on September 29, 2014, the Resident suffered another traumatic incident wherein he fell striking his head against a well. He suffered blunt force trauma to his head and was negligently placed back into his bed without proper neurological assessment wherein hours later he was found dead.

RESULT: Case is on-going.

March 30, 2016: LAYNE, WILLIAM B. V SOVEREIGN HEALTHCARE OF WEST PALM BEACH, LLC
Allegations: While at Boulevard Rehabilitation Center, the Resident was left unattended leading to her suffering a significant unsupervised fall resulting in a fracture her right hip, requiring surgery. She also suffered skin breakdown. Due to the violations by the Facility, the Resident wrongfully died.

RESULT: Case is on-going.

August 5, 2015: HARRIS, LILLIE B V SOVEREIGN HEALTHCARE OF WEST PALM BEACH LLC
Allegations: The Resident suffered a preventable fall at Boulevard Rehabilitation Center due to the Facility’s negligence, which resulted in serious bodily injury including a fractured hip. The violations of the Facility caused the Resident to die on October 25, 2013.

RESULT: Case was disposed by Arbitration wherein Plaintiff obtained a $300,000 Judgment on December 7, 2016.

December 30, 2011: DECICCO, CAROL V SOVEREIGN HEALTHCARE OF WEST PALM BEACH LLC
Allegations: The Resident was admitted to Boulevard Rehabilitation Center on July 13, 2010 with zero pressure sores. On July 15, 2010, the Resident had a red circular spot on her coccyx. Her skin continued to break down and she developed painful pressure sores which required surgical intervention. She was discharged from the Facility on July 20, 2010 and later died on November 23, 2010.

RESULT: Case went to trial where the Facility obtained a Defense Verdict on February 27, 2014.

October 30, 2009: ANISMAN, HELEN V SOVEREIGN HEALTHCARE OF WEST PALM BEACH LLC
Allegations: The Resident was transferred to Boulevard Rehabilitation Center on April 9, 2007. The Resident was injured due to physical therapy by putting her in positions that were not prescribed by her surgeon and by the known standards of physical therapy in the community.

RESULT: Case was sent to Arbitration and was disposed on August 25, 2015.

September 25, 2008: HUFFER, JASON V SOVEREIGN HEALTHCARE OF WEST PALM BEACH
Allegations: The Resident was admitted to Boulevard Rehabilitation Center who failed to prevent him from suffering falls, suffering the development and deterioration of infections and sepsis, suffering unexplained injuries and suffering wounds. As a result, the Resident wrongfully died on November 4, 2007.

RESULT: Case was disposed on February 10, 2011.

April 23, 2008: ROTBERT, CAROL V SOVEREIGN HEALTHCARE OF WEST PALM BEACH
Allegations: The Resident was admitted to Boulevard Rehabilitation Center who failed to properly monitor and supervise the Resident. As a result, the Resident suffered two falls, one of which was a catastrophic fall which resulted in the Resident suffering a total dislocation of his recent surgically replaced hip, which required the Resident to undergo a second total hip replacement.

RESULT: Case was disposed on October 23, 2009.

Allegations: The Resident was admitted to Boulevard Rehabilitation Center and wrongfully died on December 14, 2016 due to the Facility’s negligence which caused the Resident trauma including broken bones and her death.

Allegations: On June 24, 2005, the Resident was escorted to the restroom via wheelchair by an employee for Boulevard Rehabilitation Center. The employee left the Resident unattended for a period of time where the Resident fell off the toilet fracturing her neck and was placed in a “halo” vest and forced to use the aid of a feeding tube for the next several months, required home health care, and later died on July 15, 2006.

RESULT: Case disposed by Settlement for $20,000.00 on April 22, 2008.
