Allegations: The staff and employees at Avante at Lake Worth failed to provide appropriate care, treatment and services in response to a trigger of the emergency call light system, failed to respond in a timely fashion to an urgent respiratory problem, failed to monitor oxygen saturation, vital signs, and to perform assessments on the Resident’s breathing. As a result, the Resident died.

Allegations: The staff and employees at Avante at Lake Worth failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident and to monitor and supervise the care and treatment provided to the Resident and promptly transfer him to the hospital when his condition significantly worsened. Due to these failures, the Resident developed significant skin breakdown and bed sores, requiring medical and surgical intervention. As a result, the Resident died.

Allegations: The Resident sustained a first fall within the first 48 hours of her arrival to Avante at Lake Worth. She was rushed to the hospital and later retuned to Avante at Lake Worth as a fall risk. Following her second admission, the Resident fell again. Avante at Lake Worth did not notify the family of both falls despite the requirement to do so. Later, the Resident was found on the floor disconnected from O2, lethargic and with shallow breathing. An x-ray of her hip, pelvis and femur were performed. The x-ray results showed a left hip fracture and modestly displaced femur. The radiology report diagnosed the Resident with a left intertrochanteric fracture with mild to moderate displacement, left hip fracture, and the Resident was then transferred to JFK Hospital. She remained wheelchair bound due to this incident.
