Allegations: The Resident was admitted to Abbey Delray on May 24, 2013 following her discharge from St. Mary’s Medical Center for rehabilitative care and treatment. On June 22, 2013, the Resident after having repeatedly call for assistance and receiving no responses to her repeated calls, fell when attempting to leave her room to find help, resulting in nasal and left wrist fractures. Thereafter, due to the injury sustained in the fall, the Resident developed a stage III decubitus ulcer which was discovered at Chatsworth at PGA National on June 26, 2013. The decubitus ulcer was caused by a lack of attention and care by the nursing home staff, doctors, registered nurses, and other care providers while in the care of Abbey Delray.
RESULT: Case is on-going.
March 4, 2016: MUSIC, ADELE V LIFESPACE COMMUNITIES
Allegations: The Resident was admitted to Abbey Delray on November 24, 2014 due to a previous fall that resulted in a fractured pelvis and therefore qualified her as a fall risk. On November 30, 2014, the Resident suffered an unattended and unsupervised fall resulting in injury including a fractured wrist, elevated brain injury and neck injury.
RESULT: Case disposed on September 23, 2016.
April 21, 2015: LANDELL, SONIA V LIFESPACE COMMUNITIES INC DBA ABBEY DELRAY
Allegations: An individual slipped and fell on urine that was on the floor inside Abbey Delray which had accumulated as a result of a patient urinating on the floor. Due to the fall, the person suffered serious and permanent injuries.

Allegations: The Resident was admitted to Abbey Delray where the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of sepsis, suffering the development and deterioration of respiratory failure, and suffering unexplained injuries. As a result, the Resident wrongfully died.

Allegations: The Resident was admitted to Abbey Delray between July 11, 2007 and April 2008. The Facility failed to prevent or minimize skin breakdown and failed to stabilize the Resident’s ulcers or transfer the Resident to an appropriate wound care facility. As a result, the Resident died on April 26, 2008.

Allegations: On February 26, 2007, the Resident was admitted to Abbey Delray where he remained until April 5, 2007. During the Resident’s stay, the Facility failed to make efforts to protect the Resident who suffered falls resulting in a left hip fracture requiring left hip bipolar hemiaerthroplasty.
