Allegations: On or about June 19, 2015, the Resident was the victim of negligent and substandard care and treatment at Wilton Manors Health & Rehabilitation Center causing her to develop decubitus ulcers and other serious medical conditions resulting in her death. The Resident suffered from numerous Stage I through IV decubitus ulcers and unstageable decubitus ulcers on her lower extremities many of which became infected and a deteriorating Stage II decubitus ulcer on her sacrum. These injuries caused her death on August 10, 2015.

Allegations: While at Wilton Manors Health & Rehabilitation Center, the Resident sustained aspiration pneumonia, multiple falls resulting in a femur fracture, cervical spine sprain, and facial lacerations, skin integrity issues, including a stage III pressure sore to his right heel that became infected, a stage III pressure ulcer to his right tow and a stage II pressure sore to his right buttock, and infections, including a urinary tract infection, eye infection, and MRSA. Due to these injuries, the Resident died.

Allegations: On July 10, 2014, the Resident had a right hip replacement at Holy Cross Hospital, which was uneventful. The Resident had a seizure disorder and required use of medication. At Wilton Manors Health & Rehabilitation Center, the Resident’s seizure medication was discontinued. As a result, the Resident had a seizure, fell, and fractured his right hip and leg. The Resident had to undergo an additional surgery to his right hip due to the fracture.
