July 27, 2017: Edith Marrero Smith Plaintiff vs. FL Hud Margate LLC, et al Defendant
Allegations: While at Margate Health and Rehabilitation Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deterioration of wounds and suffering falls an unexplained injuries. Due to these injuries, the Resident wrongfully died on May 9, 2016.

July 20, 2017: Robert Kelly Plaintiff vs. Fl Hud Margate LLC Defendant
Allegations: The Resident was admitted to Margate Health and Rehabilitation Center where the staff and employees failed to develop an adequate care plan and failed to properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of injuries and pressure ulcers.

June 15, 2017: Roger R Caron, II Plaintiff vs. FL Hud Margate LLC, et al Defendant
Allegations: The Resident was admitted to Margate Health and Rehabilitation Center on March 4, 2016. During her admission, the Resident suffered serious injuries which resulted in her death after she fell on or about March 20, 2016. The Facility had knowledge that the Resident was deemed a high risk for falls but left her alone and unattended. As a result, the Resident fell and fractured her hip and was found on the floor of her room by the Facility’s staff. The Facility then inexcusably concealed the fall and nature of the Resident’s injuries. The Resident was eventually transferred to North West Medical Center where her injuries caused a swift decline in her physical and mental health and she died on March 22, 2016.

December 16, 2015: Margarita Zerquera Plaintiff vs. Fl HUD Margate, LLC Defendant
Allegations: The Admissions Development Director for Margate Health and Rehabilitation Center fell as the Facility negligently maintained the property by allowing a dangerous or slippery substance to exist on the floor. As a result, she suffered injuries in and about her body and extremities.

RESULT: Case disposed by Settlement on March 6, 2017.

June 6, 2015: Linda Manganiello Plaintiff vs. Fl Hud Margate LLC Defendant
Allegations: On July 16, 2014, an individual was visiting Margate Health and Rehabilitation Center for the purpose of rehabilitation for her spouse. The individual slipped and fell on the wet floor in the room of her husband was residing that was leaking from under the wall of an adjacent room. The Facility failed to clean an accumulation of water which caused the individual to fall and be injured.

RESULT: Case is on-going.

March 28, 2014: Ronald Keith Chue, Jr. Plaintiff vs. FL HUD Margate, LLC Defendant
Allegations: The Resident was at Margate Health and Rehabilitation from November 8, 2012 through December 1, 2012 and again from December 11, 2012 and February 11, 2013. During the Resident’s admissions, the Facility failed to properly assess and evaluate the Resident or provide skilled nursing care to prevent her from developing decubitus ulcers which later became infected. On December 1, 2012, the Resident was found to have developed a large infected pressure ulcer in her coccyx. The infected ulcer caused or contributed to cause a steep and rapid decline in her health. Due to these injuries, the Resident died on June 18, 2013.

RESULT: Case disposed on February 16, 2017.

July 15, 2013: Marcella Johnson, et al Plaintiff vs. HUD FACILITIES LLC, et al Defendant
Allegations: The Resident was admitted to Margate Health and Rehabilitation on May 14, 2012 with dementia. She was wheelchair bound and admitted with a small sacral pressure sore. She was noted to need ‘total assist’ with transfers and ambulation. Significantly, she could not walk on her own. On June 6, 2012, a full skin check was performed which showed her wound worsened and that she developed an entirely new pressure sore. On June 9, 2012 a note indicates the Resident’s daughter was less alert, which caused her to be transferred to Coral Springs Medical Center on June 9, 2012. At Coral Springs Medical Center the Resident was diagnosed with a fractured hip, a fractured pelvis and pressure sores. The fall and/or dropping incident was concealed by the facility staff for an unknown period of time. The Resident was forced to endure days of debilitating pain from her broken hip/pelvis and pressure sores. As a result, the Resident wrongfully died on May 15, 2014.

RESULT: Case disposed by Settlement on November 16, 2015.
