Allegations: On September 27, 2012 the Resident was 82 years of age and she suffered from dementia and other infirmities of aging that affected her ability to protect herself from injuries. She was also wheel chair bound as a result of bilateral knee surgery leaving her at high risk for the development of bedsores. The Carlisle Palm Beach failed to assess or reassess the Resident as an appropriate assisted living facility candidate and, as a result, the Resident suffered the development of a bedsore the to coccyx. The development and/or deterioration of bedsores resulted in severe injuries that caused and/or contributed to her death on August 10, 2014.
RESULT: Case was disposed by settlement on July 28, 2016.
March 6, 2013: FENDRICK, LEONE V SENIOR RESOURCE GROUP LLC DBA THE CARLISLE PALM BEACH
Allegations: On November 26, 2011, an invited individual at the Carlisle Palm Beach was walking through the main doorway, entryway/pedestrian access route to the lobby of the premises when she tripped due to an unexpected, unforeseen and abrupt change in elevation on the doorway threshold aluminum saddle and fell. The threshold portion of aluminum saddle was buckled and raised causing a tripping hazard on the walking surface. Due to the fall the invited individual was hospitalized and unable to attend to her usual occupation and avocation.
RESULT: Case was disposed by settlement on July 1, 2014.
