Allegations A resident at Brookdale West Boynton Beach was at high risk for pressure ulcers due to her dementia, lack of interest in daily activities, failing to leave her bed for several days and general physical health. As a result of the negligence of Brookdale West Boynton Beach, the Resident suffered from such severe and horrific bed sores she was removed from Brookdale West Boynton Beach and now allowed to return because of Brookdale West Boynton Beach’s inability to care for such wounds they caused.

Allegations: A guest at Brookdale West Boynton Beach tripped and fell while at the facility. Brookdale West Boynton Beach failed to maintain the premises and provide safety precautions to protect patrons to keep them same from hazards.

Allegations: The Resident left his room at Brookdale West Boynton Beach on the fourth floor of facility and went down to the main level. While on the main level, he exited the door to the pool area, which should have been locked and/or should have been monitored by a wanderguard/catch-all alarm. After exiting the building into the pool area, the Resident – while unobserved, unsupervised, and/or unattended by staff of Brookdale West Boynton Beach fell into the pool and drowned.

Allegations: The Resident was accepted at Brookdale West Boynton Beach even though said admission was wholly inappropriate in light of the fact she required heightened nursing care and supervision based upon her health condition. Additionally, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering falls and unexplained injuries suffering elopements, being attacked by another resident, and suffered the development and deterioration of wounds.

Allegations: While at Brookdale West Boynton Beach, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering falls and unexplained injuries. Additionally, Brookdale West Boynton Beach failed to appropriately monitor the Resident and recognize significant signs and symptoms of her health condition, such as suffering unexplained injuries and suffering falls. Due these injuries from the fall, the Resident died on May 8, 2013.
