Allegations: The staff and employees at Golfcrest Healthcare Center failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deterioration of wounds, suffering the development and deterioration of dehydration, and suffering the development and deterioration of infections and sepsis. Due to these actions, the Resident wrongfully died.

Allegations: Resident was placed in Golfcrest Healthcare Center needing help with feeding, maintaining her personal hygiene, prevent her from falling, and administer medications. Resident suffered from dementia, anxiety, diabetes, hypertension, abnormal weight loss, cachexia, abnormal gait, and general weakness, and she had become a known fall risk. During her admission, Golfcrest Healthcare Center negligently allowed the Resident to fall. She fell a second and third time. The third fall caused the Resident to suffer a fractured right hip. After her hip surgery, the Resident was brought back to Golfcrest Healthcare Center where she sustained yet another fall. The Resident also was identified as having a Stage III pressure/sacral ulcer. Due to the Resident’s suffering and in a compromised, weakened condition, the Resident prematurely died as a direct result of the nursing home’s negligence.
