January 29, 2015: WILES, BETSY V FI-PALM BEACHES LLC
Allegations: While at The Rehabilitation Center of the Palm Beaches the Resident developed a persistent rash consisting of red linear trails and red lesions all over her body. On January 7, 2013, the Resident was discharged to Good Samaritan Hospital where a biopsy of the crust revealed that she was severely and totally infested with mites known as Norwegian crusted scabies.
RESULT: Case disposed on March 11, 2016.
February 8, 2012: TOWNS, BRENDA BROWN V FI-PALM BEACHES LLC
Allegations: On February 18, 2010, the Resident was noted to not have any pressure wounds while at The Rehabilitation Center of the Palm Beaches. However, the staff failed to appropriately care for the Resident’s skin causing his skin to break down and he was noted on February 22, 2010 to have a Stage 4 pressure wound and he subsequently developed pressure sores on his right buttocks, left heel, right heel and right leg. As a result, he had to undergo multiple painful debridements on his pressure wounds and the Resident died on August 23, 2011 still suffering with his painful wounds and infections at the time of his death.
RESULT: Case disposed on September 25, 2013.
April 20, 2011: HUNTER, THERESA V FI-PALM BEACHES LLC
Allegations: While at The Rehabilitation Center of the Palm Beaches, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deterioration of hypovolemic shock, suffering sepsis and suffering severe dehydration. As a result, the Resident wrongfully died on December 5, 2010.
RESULT: Case disposed on July 31, 2012.

Allegations: While at The Rehabilitation Center of the Palm Beaches, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering falls, suffering unexplained injuries, suffering weight loss and malnutrition and suffering a fatal brain bleed due to excessive anticoagulation. As a result, the Resident wrongfully died on July 24, 2010.

Allegations: On March 28, 2007, The Rehabilitation Center of the Palm Beaches failed to continue the Resident’s intravenous medication as prescribe and prematurely and negligently discharged her on April 3, 2007 and the Resident was involved in a catastrophic fall.

Allegations: The Resident was admitted on July 30, 2005 to The Rehabilitation Center of the Palm Beaches where she lived until September 19, 2005 when she was hospitalized at St. Mary’s Medical Center from September 20, 2005 through November 15, 2005. The Resident suffered from neglect, delays in the provision of medical care, inadequate and poor pressure sore prevention and treatment measures, failure to ensure proper fluid intake and meal consumption resulting in the development of significant pressure sores, electrolyte imbalances, severe malnutrition, dehydration and weight loss.
RESULT: Case is on-going.
