Allegations: The Resident was admitted to Heartland Health Care Center – Prosperity Oaks where the employees failed to develop and adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of pressure wounds and infections. As a result, the Resident wrongfully died on July 28, 2015.

Allegations: While at Heartland Health Care Center – Prosperity Oaks in June 2006, the Resident suffered from neglect and was inappropriately treated resulting in left heel and right foot fluid filled blisters and ulcers on his feet. The pressure sores led to a lower extremity infection spreading to his knee, which led to multiple operations.
