Allegations: The Resident was admitted to Hamlin Place of Boynton Beach on October 26, 2014 where the Facility failed to implement proper fall precautions and failed to provide an appropriate level of supervision and assistance. On October 30, 2014 the Resident suffered a preventable fall resulting in an acute comminuted intertrochanteric femoral fracture and rami fractures.
