Allegations: The employees and staff of ManorCare Health Services – Delray failed to develop an adequate care plan and failed to properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deterioration of injuries including pressure ulcers that were unstageable and resulted in an infection down to his bone and ultimately ended his life.

November 16, 2016: DICKER, JEFFREY V MANOR CARE OF DELRAY BEACH FL LLC.
Allegations: A guest of a resident was injured in the parking lot of ManorCare Health Services – Delray due to a fall which resulted from a combination of hazards, including an unlit area of the parking lot near the front entrance of the facility. The guest sustained bruising, contusions to the head, including bleeding on his brain, nasal fractures, left wrist fractures and lacerations and derangement to his knees.

RESULT: Ongoing case.

November 2, 2015: ESTATE OF ESTELLE TOLMAN V MANORCARE OF DELRAY BEACH FL LLC.
Allegations: While at ManorCare Health Services – Delray, the Resident fell from her wheelchair, had multiple infections, including a chronic hip infection, wounds positive for MRSA, cellulitis, osteomyelitis, pneumonia, and a urinary tract infection. The Resident also had skin integrity issues including an abscess of her leg, a laceration to her right leg, and open areas and scabs on her arms; severe constipation; weight loss; and toxic-metabolic encephalopathy.

RESULT: Ongoing case.

May 23, 2014: LARVA, GRACE BLAIR V MANOR CARE OF DELRAY BEACH FL LLC.
Allegations: While at ManorCare Health Services – Delray, the Resident sustained a fall event which resulted in a severe hematoma. The fall and/or drop remained undocumented by ManorCare Health Services – Delray’s staff.

RESULT: Case Disposed on April 28, 2015.

May 6, 2014: SCHNELL, JUDITH G V MANOR CARE OF DELRAY BEACH FL, LLC
Allegations: The Resident was admitted to ManorCare Health Services – Delray on October 14, 2012 where she was the victim of negligent and substandard care and/or treatment at the facility causing her to develop medical conditions which caused or contributed to the causation of her death on November 25, 2012.

July 31, 2013: ESTATE OF DORA RAUCH V HCR MANORCARE INC NKA HCR MANORCARE LLC.
Allegations: While at ManorCare Health Services – Delray, the Resident sustained multiple falls with one resulting a left intertrochanteric fracture. The Resident also suffered from a rash to her groin and buttocks; malnutrition; and weight loss. These injuries resulted in the death of the resident.

RESULT: Case Disposed by Settlement: February 11, 2016.

October 21, 2010: ESTATE OF ALEXANDER HITZ V MANOR CARE OF DELRAY BEACH FL LLC.
Allegations: While at ManorCare Health Services – Delray, the Resident was negligently treated resulting in worsening of a Stage II decubitus ulcer to a Stage IV. The Resident also suffered from decreased blood pressure, uncontrolled high blood sugar, a fall from his wheelchair, septic shock, hypotension, dialysis line sepsis, and MRSA found in blood cultures. These injuries caused the Resident’s death.

RESULT: Case Disposed on August 9, 2012.

September 18, 2009: BURGER, JACQUELINE V MANOR CARE OF DELRAY BEACH FL LLC.
Allegations: While at ManorCare Health Services – Delray, the Resident sustained a urinary tract infection and sepsis and serious decline of health. The facility failed to reassess and modify the Resident’s plan of care in response to significant changes in condition including critical lab values, development and deterioration of a urinary tract infection which resulted in the Resident’s death.

RESULT: Case Disposed by Settlement: January 11, 2010.
