Allegations: ON September 4, 2015, the Resident was admitted to Barrington Terrace of Boynton Beach of Boynton Beach for rehabilitation and a history of falling. On September 12, 2015, the Resident was improperly monitored in her room when she sustained a fall from her wheelchair at the facility resulting in a broken left hip, along with other significant and permanent injuries. Afterwards, the nursing staff and/or aides at Barrington Terrace of Boynton Beach failed to sufficiently supervise, chart, and document the events of the fall. The Resident’s kin was not timely notified as to the serious nature of the Resident’s injuries. Due to these injuries, the Resident wrongfully died.

RESULT: Case is ongoing.

February 13, 2015: L HOMMEDIEU, ELSA V SNH SE BARRINGTON BOYNTON TENANT LLC DBA
Allegations: On September 10, 2014, the Resident was admitted to Barrington Terrace of Boynton Beach where she remained until being discharged on September 13, 2014. The Facility breached duties owed to the Resident which caused her to be injured including a fall resulting in a left hip fracture.

RESULT: Case disposed on July 9, 2015.

January 9, 2015: SILVER SMITH, DIANE V SNH SE BARRINGTON BOYNTON TENANT LLC
Allegations: The Resident was admitted to Barrington Terrace of Boynton Beach with dementia, a history of proper falls and required supervision and assistance with ambulation from staff at Barrington Terrace of Boynton Beach. Despite her high risk for falls, the Resident suffered numerous falls at Barrington Terrace of Boynton Beach, including two falls that occurred on July 17, 2014. Just hours after fall, the Resident was found unattended on the floor by the bathroom with a serious fracture. Due to these injuries, the Resident wrongfully died.

RESULT: Case was sent to Arbitration and disposed on September 29, 2016.

November 14, 2014: CUPPI, LORRAINE V FIVE STAR QUALITY CARE INC
Allegations: While at Barrington Terrace of Boynton Beach, the facility failed to provide adequate and appropriate staffing at the facility and failed to properly assess the Resident and formulate a care plan for her. As a result, the Resident sustained coccyx decubitus ulcers, left heel decubitus ulcers, right heel decubitus ulcers and the need for decubitus ulcer debridement at Delray Medical Center. Due to the injuries, the Resident wrongfully died.

RESULT: Case was sent to Arbitration and disposed on October 25, 2016.

June 27, 2014: ESTATE OF IRMA KISCHKOWER V SNH SE BARRINGTON BOYNTON TENANT LLC
Allegations: Barrington Terrace of Boynton Beach breached duties owed to the Resident which caused injury that included an erythematous rash to her abdomen, urinary tract infection positive for E.coli, pneumonia, sepsis (L. gasseri), septic shock, dehydration, and malnutrition.

RESULT: Case was sent to Arbitration and is on-going.

December 10, 2008: GLAZIER, JOAN V DELRAY MEDICAL CENTER INC
Allegations: On March 1, 2007, the Resident was admitted to Barrington Terrace of Boynton Beach of Boynton Beach where he was reliant upon the Facility for taking care of all of his needs. The Resident did not have any skin tears upon admission. On March 12, 2007 the Resident had three Stage 1 pressure ulcers to his left heel, an unstageable wound to his right heel, and an unstageable area to his right bunion. Due to these injuries, the Resident wrongfully died.

RESULT: Case disposed on October 8, 2009.
