December 17, 2015: Edelmira Alvarez Plaintiff vs. University Hospital LTD, et al Defendant
Allegations: The Resident was admitted to University Hospital on November 17, 2013 following a seizure. At the time of admission, she suffered from dementia, Alzheimer’s disease, was largely wheelchair bound and required assistance from staff for movement, repositioning, ambulation, and total care for all activities of daily living. Her skin was intact upon admission. On December 5, 2013, the Resident was admitted to Broward Nursing & Rehabilitation Center where she remained until January 3, 2014. The Facility failed to implement skin breakdown measures, prevent pressure sores, and maintain the Resident’s skin integrity. Due to these injuries, the Resident wrongfully died.

June 10, 2015: Esther Harris Dennis Plaintiff vs. Broward Nursing & Rehabilitation Center, LLC, et al Defendant
Allegations: The Resident was admitted to Broward Nursing & Rehabilitation Center from September 19, 2012 through July 2, 2013. The Resident was at risk for skin breakdown and for pneumonia. The Facility neglected the Resident and allowed her to suffer skin breakdowns and infections and failed to properly and timely treat her pneumonia. During her stay, the Resident developed and had worsening of pressure ulcers and infections, including a Stage III wound to her coccyx. On September 20, 2012, nursing notes indicate that the Resident was admitted with a Stage II wound to her coccyx measuring 1.8 x 1.3 x 0.2cm. On December 20, 2012, notes indicate that the Resident’s coccyx wound was now 5.5 x 2.4 x 0.2cm. On May 1, 2013, her pressure sore is documented as a Stage III. She also suffered from an infection to this wound. On April 16, 2013, the nursing notes indicate that the Resident developed another pressure to her left buttocks which measured 1 x. 0.5 x 0.2cm. She suffered from pneumonia and remained undiagnosed and untreated until she was found in respiratory distress on July 2, 2013. Due to these injuries, the Resident wrongfully died on October 29, 2015.

February 19, 2015: Juanita A Cook Plaintiff vs. Broward Nursing & Rehab Ctr LL Defendant
Allegations: While at Broward Nursing & Rehabilitation Center from May 23, 2013 through May 29, 2013 and also from June 2, 2013 through June 18, 2013, the Resident developed severe pressure ulcers, including a Stage IV pressure ulcer on his coccyx. He also suffered from severe dehydration and excess fluid retention which remained untreated for several days and contributed to the Resident suffering from acute renal failure. By June 9, 2013, the Resident’s sacrum wound had become a Stage IV, measuring 5.5 x 0.2cm and grew to 9 x 7 x 1.8cm by June 18, 2013. These conditions contributed to the Resident’s untimely death on June 21, 2013.

Allegations: While at Broward Nursing & Rehabilitation Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering falls and unexplained injuries. As a result, the Resident wrongfully died on October 16, 2014.

Allegations: While at Broward Nursing & Rehabilitation Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of wounds, suffering a dislodged feeding tube, suffering falls and unexplained injuries, and suffering malnutrition and weight loss. As a result, the Resident wrongfully died on October 10, 2012.
