Allegations: While at Stratford Court of Boca Raton, the facility breached its duties owed to the Resident which caused injuries including multiple infections, a urinary tract infection, upper respiratory infection, pneumonia, skin impairments, including a rash to the top of the head and between the legs, multiple falls resulting in injuries, including soft tissue swelling along the left frontal supraorbital region, a skin tear to the left elbow, and lacerations to the left eye, left nose, and right forehead. Due to these injuries, the Resident wrongfully died.
RESULT: Case was disposed on March 7, 2017.
April 24, 2012: ZEBNIAK, NANCY V SUNRISE SENIOR LIVING SERVICES INC
Allegations: During the Resident’s stay at Stratford Court of Boca Raton, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident, in order to prevent her from suffering falls and unexplained injuries. As a direct result, the Resident suffered unexplained injuries and numerous falls, including a catastrophic fall on December 13, 2011, which resulted in the Resident suffering a fractured hip and she eventually died on January 18, 2012.
RESULT: Case was removed to Federal Court on May 29, 2012.
June 1, 2009: ESTATE OF FRANCES DERESH V SUNRISE SENIOR LIVING SERVICES INC
Allegations: While at Stratford Court of Boca Raton, the facility breached its duties owed to the Resident which caused injuries including a decubitus ulcer on her sacrum that increased in size and severity during her dates of residency, worsening from a Stage III measuring 3.1cm x 3cm x .4cm to a Stage IV measuring 12cm x 11cm x 4cm, which also tested positive for Proteus Mirabilis and Klebsiella and became gangrenous. The Resident also suffered from multiple infections including conjunctivitis in both eyes, septicemia, septic shock and she became protein malnourished. Due to these injuries, the Resident wrongfully died.
RESULT: Case was disposed on January 18, 2011.
August 18, 2008: LASHER, STEPHANIE V SENIOR LIVING SERVICES INC DBA STRADFORD COURT OF BOCA RATON
Allegations: The Resident was at Stratford Court of Boca Raton between September 19, 2006 through November 27, 2006 where she developed pressure ulcers and skin breakdown. The Facility also did not provide proper nutrition, causing her to become severely ill, malnourished and dehydrated. Due to these injuries, the Resident wrongfully died.
