May 9, 2017: FRANCHI, JOANNE V SF ROYAL MANOR LLC
Allegations: The Resident was admitted to Royal Palm Beach Health and Rehabilitation Center when on August 11, 2016, the Resident fell and lying down on the floor. She suffered a head injury and hematoma. The records indicate that she was going to the bathroom when she fell. After this fall, the Facility failed to implement any fall prevention protocols. The Resident fell on August 31, 2016 when she was found next to her wheelchair and on November 7, 2016 she was found on the floor in her room as she was trying to get to her toilet and fell. On December 4, 2016, the Resident fell again which took her life as she suffered a subdural hematoma which resulted in her being transported to Delray Medical Center. The surgeons recommended she go to hospice where she died from her injuries.

RESULT: Case is on-going.

October 13, 2015: HARVEY, KATHLEEN V CHODAY MD, RATNA DEVI
Allegations: The Resident was admitted to Royal Palm Beach Health and Rehabilitation Center on May 26, 2013 for physical therapy following lumbar surgery. During the admission, the Resident’s white blood count (WBC) was 18.25 which was trending upwards. On June 6, 2013 the Resident was transferred to Wellington Regional Medical Center with acute sepsis and acute toxic encephalopathy. The Resident died on June 13, 2012 as a result of the foregoing sepsis.

RESULT: Case disposed on March 22, 2017.

June 4, 2014: CINCOTTA, JOHN V SF ROYAL MANOR LLC DBA ROYAL MANOR NURSING AND REHAB
Allegations: An individual was injured while exiting the Facility due to a slippery doorway which was unsafe sloped non-slip resistant. As a result, the individual fell and was injured.

RESULT: Case disposed by Voluntary Dismissal on December 2, 2014.

December 17, 2013: CHODKOWSKI, GRACE V SF ROYAL MANOR LLC
Allegations: The Resident suffered a catastrophic fall on February 22, 2013 which resulted in her suffering traumatic injuries.

RESULT: Case disposed by Lack of Prosecution on January 25, 2016.

September 4, 2013: BARRETO, ALONSO V SF ROYAL MANOR LLC DBA ROYAL MANOR
Allegations: A Resident at Royal Palm Beach Health and Rehabilitation Center was injured when an unsecured television wall mount caused a television to fall and his the Resident in the head causing a laceration and cervical injuries.

RESULT: Case dismissed on December 16, 2015.

December 13, 2012: OREAR, JAN V SF ROYAL MANOR LLC
Allegations: The Resident was admitted to Royal Palm Beach Health and Rehabilitation Center when on November 6, 2011 the Resident suffered his third fall resulting in a fractured hip which required surgery and resulting in major complications causing his death on January 19, 2012, two months after the fatal fall.

RESULT: Case disposed on July 16, 2013.

June 14, 2011: KOMAROMI, ANNA V SF ROYAL MANOR LLC
Allegations: The Resident was admitted to Royal Palm Beach Health and Rehabilitation Center when on September 9, 2010 the Resident was involved in a traumatic, preventable incident at the Facility which resulted in a fractured femur. After the incident, the Facility caused the Resident to remain in the facility with an untreated fractured femur for nearly a full week. Due to the Facility’s actions, the Resident wrongfully died on October 5, 2010.

RESULT: Case disposed on July 5, 2012.

July 1, 2010: CATHERINE COZ AS PERSONAL REP OF ESTATE OF BERNARD COX V SF ROYAL MANOR LLC DBA ROYAL MANOR
Allegations: The Resident was admitted to Royal Palm Beach Health and Rehabilitation Center where the staff and employees failed to develop and adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent the Resident from suffering the development and/or deterioration of serious injuries including, but not limited to severe infections and pressure ulcers.

RESULT: Case disposed on September 25, 2013.

February 5, 2010: HACK, FRANCES V SF ROYAL MANOR LLC DBA ROYAL MANOR
Allegations: The Resident suffered a preventable fall at Royal Palm Beach Health and Rehabilitation Center on March 23, 2009 which resulted in a fractured femur that caused or contributed to her death, she also developed preventable skin breakdown and decubitus ulcers that resulted in severe pain and suffering.
