Allegations: The Resident was admitted to Renaissance Health and Rehabilitation when on July 25, 2016 where he suffered a preventable choking incident due to the Facility’s negligence which resulted in his death. The Facility failed to administer appropriate care and treatment including CPR.

Allegations: The Resident was admitted to Renaissance Health and Rehabilitation on August 12, 2014 where the Facility failed to prevent the development and worsening of decubitus ulcers on the Resident which led to Stage IV left foot decubitus ulcers, left foot osteomyelitis, left foot gangrene requiring amputation of the left second toe, and MRSA septicemia.

Allegations: The Resident was admitted to Renaissance Health and Rehabilitation when the Resident suffered a wrongful death due to the negligence of the Facility. The Resident died on July 19, 2013.

Allegations: The staff and employees at Renaissance Health and Rehabilitation Center failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deterioration of pressure sores and suffering falls and unexplained injuries. As a result, the Resident wrongfully died.

Allegations: The Resident was admitted to Renaissance Health and Rehabilitation on November 21, 2010 when she suffered a fall in the shower due to the negligence of the Facility which resulted in serious bodily injury that caused or contributed to her death on January 24, 2011.

Allegations: The Resident was admitted to Renaissance Health and Rehabilitation on July 16, 2008. The Resident was not properly supervised, secured and/or provided assistance in his bed when he attempted to stand up and subsequently fell. One of the employees noted that maggots were noted coming from the bottom of his left leg. The Resident’s wounds and/or infections were not treated appropriately resulting in infections and maggot infestation in his left lower extremity and/or exacerbation of infections on his left foot, lower back and upper buttocks. On August 29, 2008 the Resident had his left leg amputated above the knee as a result of infection.
RESULT: Case disposed on August 9, 2012.
August 1, 2008: CARLSON, GARY V SEA CREST HEALTH CARE MANAGEMENT LLC
Allegations: While staying at Renaissance Health and Rehabilitation, the Resident developed progressive deterioration and skin decubitus ulcers, including a pressure ulcer on his buttocks that was diagnosed on December 19, 2007. The pressure ulcer had developed an infection including a polymicrobial infection.
RESULT: Case was disposed on December 30, 2010.
