Allegations: The Resident was admitted to Oasis Health and Rehabilitation Center on February 5, 2016 from Bethesda Hospital East. The Resident was free of infections, specifically urinary tract infections, and was high risk for developing such infection. Due to the Facility’s negligence, the Resident suffered a severe UTI where it caused her to become septic and required her to be hospitalized. The Resident never recovered from her septic shock and died on April 12, 2016.

RESULT: Case is on-going.

May 29, 2014: ESTATE OF VIOLETTA LOPEZ V LAKE WORTH ENTERPRISES LLC
Allegations: The Resident was admitted to Oasis Health and Rehabilitation Center on May 1, 2012 where she remained until her discharge on September 23, 2012. Due to the Facility’s negligent acts and omissions, the Resident suffered falls, multiple urinary tract infections, abscess to the right lower periapical, pneumonia, aspiration pneumonia, sepsis, mandibular molar necrosis with adjacent gingival inflammation, coccygeal decubitus ulcer with a scant amount of serous drainage, skin tear to upper back, wound to left anterior leg, bruising to bilateral legs, redness and inflammation to right leg, dehydration, malnutrition, hyperkalemia, hypernatremia, acidosis, and weight loss.

RESULT: Case disposed on March 5, 2015.

December 28, 2012: KEFALIDES, GEORGE V CARESTRONG LLC
Allegations: The Resident was admitted to Oasis Health and Rehabilitation Center when on February 14, 2012 he complained that he felt like he was not getting enough air. Nothing was done to remedy the situation and the staff did not implement the required oxygen changes or apply the assistive breathing machine at night. Ultimately, the Facility was not able to handle the Resident’s oxygen needs and he prematurely died on April 5, 2012.

RESULT: Case was disposed by Settlement on June 16, 2014.

April 8, 2010: DOMINIQUE, MICHAEL V LAKE WORTH ENTERPRISE LLC
Allegations: The Resident was admitted to Oasis Health and Rehabilitation Center on August 30, 2008 and he was a fall risk and/or became a fall risk. During his admission, the Resident fell due to the Facility’s failure to properly implement fall precautions that were reasonable and necessary. As a result, the Resident was injured.

RESULT: Case was disposed by Settlement on October 14, 2014.

January 22, 2010: ESTATE OF GEORGE A DAHMER V LAKE WORTH ENTERPRISE LLC DBA LAKE WORTH MANOR
Allegations: The Resident was admitted to Oasis Health and Rehabilitation Center on February 27m 2008 where he remained until April 30, 2008. The Facility failed to prevent the Resident from developing significant multiple decubitus ulcers which progressed to Stage IV heel ulcers and coccyx ulcer. The Facility failed to prevent falls, infections in the decubitus ulcers and urinary tract infections. Due to these injuries, the Resident died.

RESULT: Case went to trial where the Resident prevailed with $800,000.00 in compensatory damages obtained on November 15, 2012 and Punitive Damages obtained in the amount of $975,000.00 on November 19, 2012.

August 23, 2007: ERICSON, NANCY V LAKE WORTH ENTERPRISES LLC DBA LAKE WORTH MANOR
Allegations: The Resident was admitted to Oasis Health and Rehabilitation Center where the staff and employees failed prevent her from suffering the development and deterioration of a urinary tract infection and dehydration. As a result of said failures, the Resident was hospitalized and died on April 8, 2006.

RESULT: Case was disposed on June 17, 2009.

July 26, 2007: ANDERSON, ARDEANIA M V LAKE WORTH ENTERPRISE LLC DBA LAKE WORTH MANOR
Allegations: The Resident was at Oasis Health and Rehabilitation Center from August 18, 2005 to August 24, 2005. The Facility failed to prevent the Resident from aspirating and failed to monitor her respiratory status. As a result, the Resident wrongfully died.

RESULT: Case was disposed on August 3, 2009.

Allegations: The staff and employees at Oasis Health and Rehabilitation Center failed to develop and adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering the development and deterioration of wounds, suffering severe dehydration and suffering unexplained injuries. Due to these injuries, the Resident wrongfully died on February 15, 2006.

RESULT: Case was disposed on October 30, 2008.
