Allegations: On June 17, 2014, the Resident was admitted to North Lake Care Center and was assessed as a high risk for pressure sores upon her admission. Within the first 6 weeks, the Resident developed a Stage 2 pressure wound of right ischium that required surgical debridement of subcutaneous tissue. Thereafter, proper precautions were not put in place to prevent sores developing again in the future as she continued to develop various pressure sores including Stage 3 pressure wounds of her right and left buttock. She also suffered an overdose of pain medication as she had two Fentanyl patches placed on top of one another causing her to be rushed to Palm Beach Gardens Medical Center. She later wrongfully died on February 10, 2015.
RESULT: Case is on-going.
November 5, 2013: BOSTON, KNOVELLA V LAKE PARK SNF LLC
Allegations: The Resident was admitted to North Lake Care Center in June 2012 where the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent the Resident from suffering the development and/or deterioration of serious injuries including infection, pressure ulcers, dehydration, weight loss, and general neglect. Due to the substandard care, the Resident was forced to undergo painful surgery including the amputation of both legs in October 2013.

Allegations: On March 26, 2011, the Resident was recovering at North Lake Care Center from a left hip replacement surgery and was at high risk of falling. Allegations: During the Resident’s stay at North Lake Care Center the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent him from suffering a Verapamil overdoes which caused his death on December 1, 2007.
RESULT: Case disposed on March 28, 2011.
February 20, 2007: STEWART, MARY V OSTROFF, RONALD
Allegations: The Resident’s leg became infested with maggots due to the negligence of the prior owners and operators of North Lake Care Center. The case went to trial on May 22, 2006 and the Resident obtained a $1.3 million judgment against the Facility; however, the Facility became asset-less and ceased business completely. The Resident brought suit to enforce the judgment due to the Facility’s fraudulent transfer of assets.
RESULT: Case disposed by Settlement on October 26, 2007.

Allegations: On October 1, 2004, the Resident was admitted to the Facility until her discharge on October 19, 2004. During the Resident’s admission, the Resident’s left leg wound became infested with live maggots resulting in left lower extremity amputation.
RESULT: Case went to trial and the Resident obtained a $1.3 million judgment against the prior owners and operators of North Lake Care Center on May 22, 2006.
