Allegations: While at Medicana Nursing and Rehab Center, the Facility failed to develop an adequate care plan and properly monitor and supervise the Resident in order to prevent him from suffering the development and deterioration of aspiration pneumonia which caused his death on September 5, 2016.

Allegations: During the Resident’s stay at Medicana Nursing and Rehab Center, the Facility neglected the Resident causing severe bodily injuries, including but not limited to a Stage IV pressure sore, infection and sepsis. Due to the violations by the Facility, the Resident wrongfully died on May 10, 2015.

Allegations: The Resident was admitted to Medicana Nursing and Rehab Center and the Facility did not ensure his safety, denied his access to adequate and appropriate health care consistent with the policies and procedures by the facility. As a result, the Resident suffered from starvation, dehydration, infections, and decubitus ulcers (bed sores).

Allegations: While at Medicana Nursing and Rehab Center, the Resident was transferred to the hospital where she was found to be septic with MRSA from an infection obtained at the Facility which resulted in her death on March 19, 2015.

May 27, 2014: FARR, ANN MARIE V SOVEREIGN HEALTHCARE OF MEDICANA LLC
Allegations: The Resident suffered a preventable fall at Medicana Nursing and Rehab Center sometime between May 24, 2013 and May 29, 2013 which resulted in a fractured hip which required surgery, hospitalization, and permanent injury to the Resident.

RESULT: Case was disposed by Settlement on May 18, 2017.

May 7, 2011: STARK, BONNIE V SOVEREIGN HEALTHCARE OF MEDICANA LLC
Allegations: An individual providing hairstyling and cutting services to Residents at the Facility was injured when a chair she was sitting in collapsed causing her to violently strike the ground.

RESULT: Case went to trial where the injured party obtained a $259,929.66 which was paid by the Facility as of December 17, 2015.

August 19, 2010: GLEASON, MONA V SOVEREIGN HEALTHCARE OF MEDICANA LLC
Allegations: The Resident was at Medicana Nursing and Rehab Center where she developed worsening of a small buttocks tears and/or abrasions she developed while at JFK Medical Center. The small buttocks tears and/or abrasions developed into a Stage IV pressure sore on the Resident’s buttocks, resulting in infection. The Resident also suffered from malnourishment which ultimately led to his death on September 23, 2008.

RESULT: Case was sent to Arbitration and was disposed on December 1, 2014.

January 21, 2009: ESTATE OF RITA M RIENDEAU V SOVEREIGN HEALTHCARE OF MEDICANA LLC
Allegations: The Resident was admitted to Medicana Nursing and Rehab Center on March 21, 2007 and remained there until April 4, 2007. The Facility breached duties owed to the Resident which resulted in a urinary tract infection, hyponatremia secondary to dehydration, pneumonia, severe weight loss, chronic renal insufficiency, severe sepsis, and respiratory failure. Due to these injuries, the Resident wrongfully died.

RESULT: Case was disposed on August 4, 2014.
