Allegations: The Resident was admitted to ManorCare Health Services – Boynton Beach on July 18, 2014 for assistance with activities of daily living. After her admission, the Resident’s condition deteriorated as she was unable to ambulate at all and was totally wheelchair bound. She required total assistance from staff for any kind of movement. She could not get out of bed without staff assistance. On May 2, 2016, the Resident’s family received a call from VITAS informing them that their mother was screaming in pain. The family rushed over and confirmed acute pain in her right leg. The doctor performed an x-ray which confirmed the Resident suffered a fracture in her right femur, specifically a comminuted fracture of her right femur due to trauma. This injury occurred due to an unsupervised fall. Due to her injuries, the Resident eventually wrongfully died on March 12, 2017.

Allegations: While at ManorCare Health Services – Boynton Beach, the Resident suffered from an unreported fall and later developed new pressure sores and had worsening of existing pressure sores on her body. Resident also had a French Foley catheter inserted incorrectly causing a significant permanent injury, including blood in her urine. Eventually the resident died from these injuries.

Allegations: The Resident was admitted to ManorCare Health Services – Boynton Beach on March 15, 2012 where he remained until being discharged on March 26, 2012. Due to the Facility’s breach of duties, the Resident suffered injury including chronic anemia, IV needle device undiscovered by staff, untreated IV site resulting in cellulitis. These injuries caused the wrongful death of the Resident on March 26, 2012.

Allegations: Resident was a bipolar schizophrenic mentally disabled adult who was wheelchair bound. The resident sustained a single Stage II pressure sore on her coccyx. The resident was told to get meticulous wound care and interviewed multiple facilities before deciding on ManorCare Health Services – Boynton Beach. The patient developed worsening of her existing pressure sores, including gaping pressure sores on her right ischium, her left ischium, her coccyx and her right heel.

May 11, 2011: RICHARDSON, CHRISTIAN V MANOR CARE OF BOYNTON BEACH FL LLC.
Allegations: Resident was found in her bed by her son with a massive head injury and subdural hematoma that had been undiagnosed and treated by the staff of ManorCare Health Services – Boynton Beach. The resident lived in a vegetative state until she was taken off life support and died the following day. Her death was identified as a subdural hematoma secondary to a probable fall.

The resident also sustained infections due to catheter site infections and complications. There was a failure to monitor midline catheters. These infections caused the resident’s death.

Allegations: Resident had undergone a series of medical procedures at Delray Medical Center, including a total abdominal hysterectomy. She needed to recovery from the surgery and need post-surgical rehabilitation at a step-down facility. She was transferred to ManorCare Health Services – Boynton Beach where she needed close monitoring of her weight and nutrition. The resident was provided breakfast only 8 days out of her 12 day stay. Of those meals consumed, she only had 75% of her meal on only one day and only 25% of her other meals. As a result of her extreme malnutrition, the resident suffered a spontaneous fracture to her lower back which the staff never recognized or addressed. The resident eventually was transferred to Bethesda Memorial Hospital were she later died from the above injuries.

Allegations: Resident was provided a wheelchair by ManorCare Health Services – Boynton Beach and was placed into a motor vehicle with the wheelchair. The driver of the motor vehicle took a sharp u-turn causing the resident’s wheelchair to fall over resulting in serious personal injuries and ultimately the death of the resident.
