Allegations: The Resident was admitted to Consulate Health Care of West Palm Beach on October 30, 2014. ON November 8, 2014, the Resident was transferred from the Facility to a hospital and found to be suffering with a deep, infected left heel decubitus ulcer which was not even assessed or documented on at least 8 dates during the short stay at the facility. While at the hospital, the Resident was found to be septic with a severe infection from which she was unable to recover succumbing to hear death on February 15, 2015.

RESULT: Case disposed by Settlement on July 13, 2017.

March 1, 2013: ESTATE OF MARY J DUPUIS V WEST PALM BEACH FACILITY OPERATIONS LLC
Allegations: The Resident was admitted to Consulate Health Care of West Palm Beach on April 12, 2011 where she remained until she was discharged on October 13, 2011. Due to the Facility breaching duties owed to the Resident, she suffered multiple infections including MRSA, Candida esophagitis, C-Diff, thrush, sepsis, and numerous urinary tract infections, numerous skin integrity issues including the development of an ulcerous lesion under her PEG tube, dermatitis to her groin and peri areas, redness to her buttocks, and an unexplained skin tear to her left hand, dehydration, malnutrition, diarrhea, and weight loss. Due to these injuries, the Resident wrongfully died on October 15, 2011.

RESULT: Case disposed on June 29, 2016.

July 23, 2012: ALZATE GIL, LUIS V WEST PALM BEACH FACILITY OPERATIONS LLC
Allegations: The Resident was admitted to Consulate Health Care of West Palm Beach in 2008 where he remained until his final discharge in May 2011. The Facility failed to provide the Resident with adequate and appropriate care which resulted in skin breakdown and infection. The Resident developed a Stage IV ulcer to his right foot which led to osteomyelitis and nearly required amputation.

RESULT: Case was disposed by Settlement on March 21, 2014.

July 5, 2012: BERGSMAN, ELIZABETH A V WEST PALM BEACH FACILITY OPERATIONS, LLC
Allegations: The Resident was admitted to Consulate Health Care of West Palm Beach on October 7, 2009 where she remained until she was discharged on October 18, 2009. Due to the Facility’s breach of duties owed to the Resident, she suffered delay in treatment, a perforated colon and sepsis which caused her death.

RESULT: Case was disposed on December 4, 2014.

Allegations: The Resident was admitted to Consulate Health Care of West Palm Beach on January 17, 2009 where she suffered the development of preventable skin breakdown and decubitus ulcers that resulted in severe pain and suffering, infection and death.

Allegations: An employee of Consulate Health Care of West Palm Beach negligently secured the Resident who was in a wheelchair in a van. When transporting the Resident, she was injured and received bodily injuries which resulted in her death on April 5, 2008.
