June 29, 2017: Deena Accardi Plaintiff vs. Tamarac Rehabilitation and Health Ctr Inc Defendant
Allegations: While at Tamarac Rehabilitation and Health Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering falls, despite the fact that she was known to be at high risk for falls. Further, there was a delay in contacting her family and treating physicians following a fall. Finally, there was a delay in transferring her to the hospital to treat her traumatic injuries following a fall. As a result, the Resident wrongfully died on February 16, 2017.

Allegations: Tamarac Rehabilitation and Health Center failed to timely transfer the Resident to a hospital, failed to timely answer a call button, failed to timely observe the Resident in her room, failed to timely call 911, failed to take measures to prevent harm. As a result, the Resident suffered a fractured skull, subdural hematoma necessitating an emergency craniotomy and ultimately death.

Allegations: While at Tamarac Rehabilitation and Health Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of skin breakdown despite the fact that she was known to be at high risk for skin breakdown. Further, there was a delay in contacting her family and treating physicians when she began to experience skin breakdown. Finally, there was a delay in transferring her to the hospital to address her skin breakdown. As a result, the Resident wrongfully died on June 24, 2016.

March 14, 2017: Rachel H Cohen Plaintiff vs. Tamarac Rehabilitation & Health Center Inc , et al Defendant
Allegations: On August 20, 2015, the nurse’s notes at Tamarac Rehabilitation and Health Center revealed that the aide reported to her at 6:30am that morning and that the Resident complained of pain. The nurse went in to evaluate the Resident and it was noted that she stated she got out of bed, went to the bathroom by herself and fell on her way back to the bed. The Resident stated that she got back to bed by herself. X-rays ordered by physician revealed a left hip fracture and wrist fracture. The Resident was transferred to Broward Health Coral Springs on August 20, 2015. Her left leg was found to be externally rotated, shortened and tender. On August 24, 2015, she underwent cemented bipolar hip replacement.

Allegations: While at Tamarac Rehabilitation and Health Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of infections and sepsis and to prevent her from suffering the development and deterioration of aspiration pneumonia. As a result, the Resident wrongfully died on September 8, 2015.

Allegations: While at Tamarac Rehabilitation and Health Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of infections and sepsis and to prevent her from suffering the development and deterioration of infections and sepsis. As a result, the Resident wrongfully died on December 16, 2015.

April 30, 2015: Phyllis Miller Plaintiff vs. Millennium Health Systems, LLC, et al Defendant
Allegations: The Resident was admitted to Tamarac Rehabilitation and Health Center on September 12, 2014 and he was a fall risk as he was recovering from an aortic aneurism endovascular repair. The Resident was the nursing home for rehabilitation following his surgery and for increased supervision. On September 13, 2014, the Resident suffered a debilitating fall just one day after his admission as he hit his head and damaged his teeth in the fall. He was rushed back to Westside Regional for stitches and care relating to his wounds. The Resident suffers on-going permanent injuries from his fall. His dementia and confusion is worsened from the head injury and he had undergone painful dental procedures as a result of the damage done to his teeth from his fall.

RESULT: Case disposed by Settlement on March 24, 2017.

April 30, 2015: Hattie Jamieson Plaintiff vs. Millennium Health Systems, Inc., et al Defendant
Allegations: The Resident was admitted to Tamarac Rehabilitation and Health Center on March 19, 2013 for rehabilitation secondary to a right hip fracture. She required assistance from staff to perform her ADL’s. Upon admission to the Facility, her skin on her sacrum was intact. She remained at the Facility from March 19, 2013 until May 22, 2013 when she was re-admitted to Broward Health Medical Center. Upon admission to Broward Health from the Facility, she was seen by a wound care nurse and 7 new wounds/ulcers had developed during her residency at the Facility. These wounds required wound care and painful debridements. The wounds she developed at the Facility plagued the Resident for the rest of her life as and she passed away on November 24, 2014.

September 3, 2013: Lisa Coutoupis Plaintiff vs. HBA Mgmt, Inc., et al Defendant
Allegations: While at Tamarac Rehabilitation and Health Center, the Resident was found on the floor on May 1, 2012 in his room lying on his left side with numerous skin tears and complaints of left sided hip pain. X-ray studies were performed of the Resident’s left hip and humerus which was reported as normal. On May 30, 2012 the Resident was transported from the Facility to University Hospital and Medical Center for evaluation due to a suspected urinary tract infection. Upon his admission to University Hospital, x-rays were performed on the Resident’s left hip, revealing an undiagnosed intertrochanteric fracture of the left hip. This resulted in a more complex repair.

RESULT: Case disposed by Settlement on October 9, 2015.
