Allegations: The Resident was admitted to Manor Pines Convalescent Center on September 26, 2014 with a history of dementia and falls. On October 12, 2014, the Resident fell and was found naked on the bathroom floor. On October 16, 2014, the Resident fell a second time and was once again found in the bathroom by the Facility’s employee. Due to the October 16, 2014 fall, the Resident suffered a laceration to the back of his head, ongoing personal injury and pain and suffering.

February 2, 2017: Dianne Williams Plaintiff vs. Manor Pines Convalescent Center LLC Defendant
Allegations: While at Manor Pines Convalescent Center, the staff and employees failed to develop an adequate care plan and properly monitor and supervise the care and treatment provided to the Resident in order to prevent her from suffering the development and deterioration of infections and sepsis. As a result, the Resident wrongfully died on November 5, 2016.

September 6, 2016: Eladio Morejon Plaintiff vs. Manor Pines Convalescent Center Llc , et al Defendant
Allegations: On January 14, 2014, the Resident was admitted to Manor Pines Convalescent Center. He was there until being transferred by Fort Lauderdale Fire Rescue to Holy Cross Hospital on July 16, 2015 where he died after choking on several large pieces of food. The Resident was unsupervised and not being assisted while eating lunch when he began choking on large pieces of steak. There was little response, if any, and/or the response as inadequate allowing the Resident to choke to death. Fire Rescue revealed that they were able to remove several pieces of food with the use of forceps.

April 13, 2016: Walter M Keough, et al Plaintiff vs. Manor Pines Convalescent Ctr, LLC Defendant
Allegations: While at Manor Pines Convalescent Center, the Resident was gravely injured on April 22, 2014. The Resident was admitted for routine rehabilitative care following left knee surgery at Holy Cross Hospital in Fort Lauderdale, Florida. Prior to his fall, the Resident was unable to get out of bed without more than one person assist. On April 22, 2014, the Resident called for the assistance of the Facility’s skilled nursing employees in order to get out of bed and use the lavatory in his room. A sole employee responded and directed the Resident to the lavatory using a wheel chair without “two-person assist.” After he had finished using the lavatory, while rising from the toilet at the employee’s direction, the Resident felt his left knee buckle. He had tried to stabilize himself by both reaching out to the employee for support and grabbing a safety bar in the restroom. However, the employee had negligently draped the Resident’s garments on the safety bar and when he attempted to grab the safety bar, he grabbed the loose garment which slipped off the bar. The Resident fell and re-opened the existing surgical wound to his left knee. He was rushed to the emergency room of Holy Cross Hospital where he almost bled to death. He was re-admitted to Holy Cross Hospital where he received multiple surgeries and is now permanently disabled.

Allegations: While at Manor Pines Convalescent Center from October 15, 2014 through May 1, 2015, the Resident developed a Stage IV sacral decubitus ulcer and died on May 1, 2015.
