Allegations: On May 26, 2015, the Resident was admitted to Hillcrest Health Care and Rehabilitation Center for rehabilitation from a recent stay at Memorial Regional Hospital where he underwent an angiogram with peripheral stent placement. Upon admission, the Resident was deemed to be a fall risk. On May 28, 2015, the Resident sustained a fall while unsupervised in his room. At 6:00am, a nurse at the Facility found him laying on the floor next to this bed, with a laceration to his head, swelling, and bleeding. The staff failed to alert emergency personnel of the Resident’s head injury and declining condition. At 9:45am, paramedics dispatched to treat a different patient were made aware of the Resident’s condition by happenstance. By the time the Resident arrived to the hospital, he was unresponsive and a CT Brain Scan was performed which revealed a worsening and extensive acute subdural hematoma. He died from his head injury later that day.

Allegations: The Resident was at Hillcrest Health Care and Rehabilitation Center from May 18, 2014 and June 27, 2014 mainly because of the infirmities of aging, pressure sore, dementia and a recent hip replacement. During the Resident’s admission, she developed a Stage IV pressure sore, deterioration of her mental state, a raging infection, and a subsequent and permanent removal of her hip replacement. She also was malnourished and heavily medicated. She was ultimately transferred and discharged by ambulance from the facility with an unstageable wounds to her hip.

Allegations: On January 6, 2014, the Resident was admitted to Hillcrest Nursing and Rehabilitation Center where she remained in primary residence until she was discharged on February 7, 2014. Due to the Facility’s breach of duties owed to the Resident, she suffered multiple falls with injury, including an upper left arm skin tear and a right hip fracture requiring hospitalization and open reduction and internal fixation surgery, fractures unrelated to falls, including an acute thoracic compression fracture and a sacral insufficiency fracture, numerous infections, including a urinary tract infection with leucocytes, Escherichia coli, and blood requiring hospitalization and oral candidiasis, and aneurysmal dilation of the abdominal aorta requiring hospitalization. As a result, the Resident wrongfully died on March 17, 2014.
